Provo Fire Marshal Identifies Manufacturing Defect in Stadium of Fire Explosion

The Provo City fire marshal identified a manufacturing defect as the cause of the catastrophic firework explosion during the Stadium of Fire event on July 4, 2024. The malfunction sent fireworks into the crowd, injuring at least 27 people, including six who required hospitalization. Investigators found that the issue stemmed from an internal flaw in the "whistle composition" of the Howling Tails of Hammer firework tubes, leading to uncontrolled explosions. Fire Marshal Lynn Schofield has since banned whistle-based fireworks inside the stadium and recommended increasing safety distances, improving product labeling, and establishing a national reporting system for firework incidents.
